Lessons to Learn from the Leaders

Wow. From the mountain of nominations we received this year, it’s clear that IT and technological advances have permeated a wide range of facility types rather than just a few large, comprehensive medical centers. This year the winners stretched from 25-bed Decatur County Memorial Hospital in Greensburg, Ind. to 2,000-bed Detroit Medical Center in Michigan, and include a children’s hospital, two large imaging groups and a variety of community hospitals and academic medical centers. Several were repeat winners, but most were not.

With so many qualified facilities, this year it was more difficult to select the Most Connected Healthcare Facilities than it was last year. According to last year’s criteria, our winners would have included 39 organizations. With tighter refinement, and a closer look at categories such as the number and diversity of imaging and IT systems integration, we narrowed the field to 25. The winners are presented here in alphabetical order (the order they are listed here in no way indicates a ranking).

Connections Make All the Difference in Healthcare

A seamless path for incoming and outgoing patient information is the goal for all the facilities nominated this year. More have succeeded than ever before. The majority of the Most Connected facilities use voice recognition (76 percent), computerized physician order entry (64 percent), clinical decision support (72 percent) and structured reporting (80 percent).

And there is no shortage of information systems installed at these facilities. One hundred percent have both a PACS and a RIS. The majority (96 percent) have both electronic medical records and a hospital information system. Eighty percent have a laboratory information system, 72 percent have a cardiovascular information system and 68 percent have a surgical information system. More than half (56 percent) tell us they have a RIS-PACS.

We asked facilities how many of 20 different systems (i.e. orthopedics, critical care, etc.) share images with PACS. Sixty-four percent have more than 15 different systems sharing images with PACS. Not surprisingly for a list of Most Connected Facilities, 88 percent of this select group are more than 90 percent filmless and 92 percent perform more than 90 percent of their imaging procedures digitally.

They also have seen significant increases in the annual number of imaging procedures performed over a year ago. Most (36 percent) have experienced growth in annual imaging procedures of 6 to 10 percent and another 20 percent saw growth in the range of 21 to 25 percent. One facility experienced an increase of 31 to 35 percent.

Report turn-around times are certainly impressive too, with the fastest radiology reports ready in less than two hours at Norton Healthcare in Louisville, Ken. Baylor University Medical Center took top honors of all the submitted survey ballots with just a 10-minute turn-around time for radiology and cardiology reports and only five minutes for emergency department reports. Jean Plummer, manager, informatics, attributes those impressive numbers to voice recognition and a lot of hard work over the past four years since the technology was implemented. Many other facilities can boast of rapid turn-around times of well under 30 minutes on a routine basis. Many ballots shared stories about how improved connectivity has cut turn-around times down by days.

Health Imaging & IT also would like to issue special recognition to Specialty Networks, a top performing RHIO in Tennessee that has developed its own software solution. Centers where Specialty Networks software has been implemented have enjoyed a reduction of 80 percent of transcriptionists and approximately 20 percent reduction of schedulers. The radiologists’ productivity has increased by 27 percent and the report turn-around time has decreased to less than an hour, down from an average of 24 hours.

U.S. healthcare organizations spent $19.1 billion on IT in 2006 and will spend $20.4 billion this year. By 2010, that number will increase by nearly 25 percent to $25.5 billion.

HIT investment is one of the few issues both Democrats and Republicans agree on, with President Bush, and presidential candidates Sen. Barack Obama and Sen. Hillary Clinton among others all calling for reduced expenditures and improving care through more electronic systems. Bipartisan bills on HIT are on the table in both the House and Senate. 

While most facilities report an impressive return on investment, improved quality of care for patients is the biggest benefit of all.

The Fine Print
This year’s online nominations were accepted from May 4th through June 8th, 2007. Some 600-plus healthcare facilities were represented in the balloting. The ballot included 45 questions. Among the factors  differentiating the winners were percentage of facility that is filmless, percentage of procedures that are digital, the number of modalities and departments that share images, and the ability of in-network physicians and referring physicians to access electronic images.

Baptist Health saved almost $1.5 million in fiscal year 2006 thanks to technology investments. All images are available online through a stand-alone PACS application or directly from patients' charts, saving time and reducing the need for duplicate tests. The dietary ordering, inventory management and medication administration all are performed online. The organization has experienced an 85 percent reduction in medication order turn-around time. The GetWell:) Network (GWN) provides internet access and communication technology to patients' hospital rooms through the television by using a special remote and a wireless computer keyboard. Patients can order movies, check email and watch educational videos about medical conditions.

A regional health system of four hospitals and 25 community health centers serving residents of a nine-county region in rural, central New York, Bassett Healthcare went filmless in April 2007. Bassett has provided comprehensive patient-centered healthcare since the 1920s and has had a formal affiliation with Columbia University College of Physicians and Surgeons since 1947. Given its 5,000-square-mile coverage area, the ability of physicians to access images via PACS in minutes positively impacts clinical care.

Boca Raton Community Hospital

Boca Raton Community Hospital offers a full array of healthcare services, along with a clinical research center and cardio/pulmonary rehabilitation center. A new $75 million, 98,000-square-foot facility to open in 2008 will house oncology imaging, radiation, chemotherapy, support services and clinical research. The hospital will become the Charles E. Schmidt Medical Center in 2011, the first new academic medical center to open in the U.S. in nearly 30 years. The 530-private room teaching hospital will become a standard for hospital safety by improving visibility of patients to staff, standardizing and automating certain tasks and procedures and minimizing physician fatigue. The new academic medical center will be the teaching hospital for the University of Miami, Miller School of Medicine.

This 755-bed, non-profit, teaching affiliate of Harvard University Medical School is a member of the Partners HealthCare System. The multi-institutional, multi-disciplinary BW/F Medical Imaging IT program includes CPOE with decision support, speech recognition and integration into the EMR. The facility has integrated process improvement, quality and patient safety programs into Medical Imaging IT for a combined approach to process redesign and technology deployment. The facility has implemented a paging portal to alert radiologists that reports are ready for review and signature and a protocoling portal for radiologists to review and specify imaging protocols to be used by technologists in imaging studies.

Based in Fort Worth, the non-profit system has added more than $100 million in new facilities since 2000. They include a child development center, a four-floor patient pavilion and critical-care areas, fracture clinic, urgent-care center and heliport, and cardiac catheterization and cardiac surgery facilities. Due to investments in CR and DR, four radiologists can read 10,000 procedures a month. Implementation of digital imaging in the NICU resulted in procedure workflow decreasing by 50 percent with CR and 75 percent with DR. The organization achieved Magnet status for nursing excellence in 2006.

Decatur County Memorial Hospital

imageDecatur County Memorial Hospital opened in 1922 as a memorial to the 800 men and women in the Decatur County area who served in World War I. This small, rural facility performs 30,000 imaging procedures a year with just one radiologist. A web-based RIS has increased the radiologist's productivity by about 30 percent and reduced film costs by 90 percent. The system also makes it easy to send certain studies off-site to sub-specialty radiologists for interpretation. Plus, an off-site x-ray facility sends images into PACS via fiber network. After implementing CR, report turn-around times decreased from eight hours to four hours. The hospital also recently installed a new digital mammography system and the radiologist can read everything on one diagnostic workstation.

imageDMC is the largest healthcare provider in southeastern Michigan with 2,000 beds and 3,000 affiliated physicians. It also is the teaching and clinical research site for Wayne State University's School of Medicine. The facility has used health information technology to increase staff productivity, reduce costs and improve patient care experience. A recent upgrade to its wireless IT infrastructure was performed to support a full-scale EMR implementation. Less than eight months after project initiation, wireless capabilities now cover more than 4 million square feet through 900 separate access points. Wireless carts and scanners verify and deliver medications, wireless internet is available for patients and their families, and mobile robots enable doctors to make rounds and interact with patients from remote locations.

imageMaimonides Medical Center trains more than 400 residents and medical students each year and routinely contributes to the progress of medicine through its broad base of clinical and laboratory research. Its IT systems have allowed for a 20 percent higher volume of studies without increasing staff, 24/7 radiology coverage with two-thirds done remotely, and file room space reduced by 90 percent and staff by 75 percent. The facility can bill for 100 percent of its studies. Maimonides uses data mirroring and server clustering for effective redundancy and disaster recovery, and is currently in the process of upgrading the network backbone infrastructure to provide multi-gigabit bandwidth to accommodate increasing volume and distribution.
